I agree that I am healthy and fit to train, and…

  1. Have not had a fever in the past 24 hours above 37.8o C (100o F)?
    Note: NMAA reserves the right to measure the temperature of all students on entry to the venue as a precaution.
  2. Have not, or recently had, any of the following symptoms: Cough, Fever, Headache, Loss of /smell, Loss of Taste, Muscle Aches, Shortness of Breath, Sore Throat, Vomiting or Diarrhoea?
    Note: Students are to inform NMAA instructor’s immediately when the symptoms above are experienced at any time.
  3. Have not been in contact with anyone in the past 14 days that has symptoms or been diagnosed with COVID-19?
    Note: Students (family) are to inform NMAA instructor’s immediately in the advent of the above situation occurring.

COVID-19 Information

COVID-19 is a contagious virus that spreads from person to person. In addition to our regular cleaning policies and procedures that this venue has in place, additional preventative measures are being followed to further reduce the spread of this novel coronavirus. However, please know that these best practices still offer no guarantee and there is a potential risk of becoming infected.

Consent for Martial Arts Service

I understand that the martial arts service provided includes physical touch and close proximity over an extended period of time, typically up to an hour. Because of this, there may be an elevated risk of infection which may include COVID-19. By agreeing to this form, I acknowledge that I am aware of the risks involved from receiving the service of martial arts instruction and training at this time. I voluntarily agree to assume those risks, and I release and hold harmless this martial arts academy / business from any and all claims related to receiving service. I give my consent to receive the martial arts service from this academy / business.

Note: It is understood and agreed that it is the responsibility of the student / parent or guardian to ensure that the venue is updated immediately of any changes in circumstances.
